Arrival Instructions

For In-Office Evaluation Appointments Only

• Remember to check-in by using the email with the subject "Check In Online For Your Upcoming Appointment". There will be limited access to check-in options within the building otherwise.
• Please arrive no more than 5 minutes prior to your scheduled appointment time.
• Park on the Northeast side of the building in the designated Cold+Flu Clinic parking spaces.
• Enter the building through the Northeast entrance.
• Only patient's should enter the building unless unable to do so on their own. If assistance is needed, only one accomanying adult is permitted.
• All patients and accompanying adults must wear protective face coverings.

For Lab Appointments Only

• Remember to check-in by using the email with the subject "Check In Online For Your Upcoming Appointment". There will be limited access to check-in options upon arrival otherwise.
• Please arrive no more than 5 minutes prior to your scheduled appointment time.
• Park on the North side of the building facing 5th Avenue in the designated Cold+Flu Clinic LAB parking spaces.
• Please stay in your vehicle. Do not enter the building.
• You will have received a text with check-in instructions, please respond to that text with the make and color of your vehicle.
• All patients must wear protective face coverings until instructed to remove them.
• One of our lab staff members will meet you at your vehicle.
