Release of Information
The Walla Walla Clinic respects your privacy. We understand that your personal health information is very sensitive. We will not disclose your information to others unless you tell us to do so, or unless the law allows us to do so.
Processing the release of records is offered when coordinating care between the Walla Walla Clinic and another provider or facility. This process will take no more than 15 days, and there is no processing fee.
To request records from another facility, please:
- contact that facility and ask them to fax the records to us at 509.525.6897, or
- visit our Walla Walla Clinic Medical Records department at the address below, to fill out a Release of Information request, or
- print and completely fill out this Release of Information Form. Note: Some outside facilities may not accept electronic signatures on this form as authorization. If so, you will be contacted to visit the Walla Walla Clinic to provide a physical signature.

If you are requesting records for your personal use, please visit us at the Walla Walla Clinic, Release of Information department, at the address above. The printing of these documents will incur a processing fee. You may also print and completely fill out this Release of Information Form and bring it with you to the Walla Walla Clinic Release of Information department, though please postpone signing the form until you are meeting with our Release of Information Clerk.
Selective records may be obtained from your FollowMyHealth Patient Portal account without visiting the Walla Walla Clinic. If you do not have access to a FollowMyHealth Patient Portal account, please visit any of our check-in desks at the Walla Walla Clinic for more information.
